Heart 2 Hearts Home Care understands that each of our clients has his or her own needs. Our care coordinator makes sure that we are able to provide care that best suits the various needs of our clients. This is done by conducting a home assessment and having a discussion with the client’s family members so we can take into consideration their preferences, as well as the client’s, when it comes to receiving care.
Throughout the time that the client is under our care, our nursing staff will provide follow-up evaluation of the client’s situation so we can customize our care plan and meet the client’s needs.
Our nursing staff will also be the ones to determine what services the client qualifies for.
